In the following example we will see a sample request letter to fill out assessment form. Some modifications are required to make your own letter !!

Dear Ms. Sterquist,

We are pleased to inform you that the New Haven Sports commission is conducting an assessment of the athletic projects supported by the Williams-Gent Foundation and SkyData Organization covering the period 2006 to 2013. The results of the evaluation and survey shall help the agency focus its thrusts on community athletic programs for the next planning period.

As your partner in community team building and sports, we would like to request your cooperation by filling up the attached questionnaire pertinent to New Haven Sports commission programs. The outcome of the survey from your respective grants will be forwarded to the steering committee.
